Provisions of Agreement

Common (general, hospital and agency for inviting of patients)

1. In the event of registering past medical history for consultation with hospital or agency
   • Member’s past medical history will be stored in the system only for 90 days following registration in the system, and will be automatically deleted thereafter. Even if the past medical history is deleted, it can be used by registering it again. When it is registered again, it will also be stored for 90 days, just like the initial registration.
   • Member must start consultation with hospital within 90 days of registration of the past medical history.
   • Once the past medical history has been copied by the hospital or agency, hospital or agency will manage this copy. As such, even if the member deletes his/her past medical history, the past medical history copied by hospital or agency will not be deleted. Whether to maintain or delete the copied past medical history will be determined by the corresponding hospital or agency.
   • Personal information of the member will be provided to the hospital or agency with which the member had consultation in the system, and the responsibility for the management of such personal information is with the corresponding hospital or agency. Personal information of the member will not be provided to any other 3rd party. However, for the purpose of translation of the member’s past medical history, translation works can be carried out by companies specializing in medical tours that the Korean government has acknowledged, rather than the corresponding hospital in accordance with the situations and policies of the hospital with which the member had consultation. Moreover, translated information on past medical history can be used when the member is engaged in consultation with another hospital. As an exception, personal information can be disclosed to the corresponding authority in the event of request for such disclosure of information in accordance with the legal execution of law for each of the countries. In addition, data with the exception of information for identification of an individual can be collected, processed and disclosed to outside for the purpose of compilation of statistical data. (For example, information on nationality, gender, age, disease and hospital visited for the purpose of compilation of statistical data.)
2. You can join membership under a false name.
   • You can join membership under a false name and we do not require provision of any information that can be used to identify you, including passport number and social security number, whatsoever.
   • However, we verify whether the e-mail address registered is the correct e-mail address of the member for notification of important information to the member.
3. The registered member information can be deleted for the member who has not logged in over a prolonged period of time.
   • In the case of a member who has not logged in the system not even once for more than 3 years, all the information registered at the time of joining membership including ID may be deleted.
   • Once the ID is deleted, the corresponding ID can be used by others.

In the case of a professional member (hospital and company inviting the patient)

1. In the case of registering a hospital or company inviting the patient, there is a need to consent not only to this provision but also the additional provisions separately. Consent to additional provisions will be processed when registering the hospital or company inviting the patient.
2. Hospital or company inviting the patient will be proceeding with joining membership through 2 stages. The first is joining as an ordinary member and the second is registering the hospital or company inviting the patient.
